Home Health Care Business Development Client Advocate Representative

- Develop and execute strategic marketing and referral development plans to grow our in-home care census.
- Establish and nurture relationships with physicians, hospital discharge planners, skilled nursing facilities, assisted living communities, and other referral sources.
- Conduct professional presentations, in-services, and community outreach events to promote services.
- Track and report on referral activity, market trends, and business development metrics.
- On-call responsibilities for clients and rotating weekend on-call schedule shared with scheduler.
- Most be able to work in a team environment.
- Serve as the first point of contact for prospective clients and families, explaining our services and conducting initial in-home care assessments.
- Provide education and emotion support to help clients and their families make informed care decisions.
- Coordinate with our scheduler and caregiver teams to ensure timely start of care dates and seamless service delivery.
- Advocate for client’s needs and educate about skilled services like home health, palliative and hospice to ensure they have the best care plan between non-medical and skilled.
- Bachelor’s degree in marketing, healthcare administration, social work, or related field preferred (or equivalent experience).
- Minimum 2 years of experience in healthcare sales, hospice, home health, or in-home care.
- Proven track record in business development, sales growth, or referral generations.
- Prior experience in patient advocacy, case management, care liaison or care coordination a must.
- Bring to our agency existing referral partners in the health care field is a plus.
- Strong interpersonal, presentation, and relationship building skills
- Ability to approach sensitive conversations with compassion, respect, and professionalism.
- Self-motivated with strong organizational and time management skills.
- Reliable transportation and ability to travel within Maricopa County territory.
